Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of being crushed by a vertical reciprocating conveyor carriage platform equipped with a safety cam mechanism that was not maintained and inspected on a regular basis as required in the owners Operating Manual and ASME/ANSI B20.1 Section 6.21 (a). AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE METHODS TO CORRECT THIS Hazard IS: To begin a schedule of equipment inspections and testing by a competent person every 250 hours of use. (b) Ensure that the vertical reciprocating conveyor meets the application requirements for the design, inspection, maintenance and operation as prescribed in ASME/ANSI B20.1. a) Location: 1st floor, plant area at 360 Beacham Street, Chelsea, MA: On or about March 13, 2009, the employer did not ensure that the vertical reciprocating conveyor's platform's safety cam mechanism was tested and maintained as recommended by the equipment manufacturer.
