North Carolina General Statute 95-129(1) of the Occupational Safety and Health Act of North Carolina: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to ionizing radiation: a)facility, on January 13, 2009, an employee performed source retrieval activities, following a failed Iridium-192 source change, and had not been properly trained. Among other methods, one feasible and acceptable method to correct this hazard is to provide source retrieval training to employees that may perform these activities.
