Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of being struck-by in case of failure of the wire rope held by only one wire rope clip or crushed by the bulldozer if failure of the wire rope occurred: (a)On or about 03/30/09 - Hwy 52 Pelham, AL, a 1 1/8-inch wire rope being used to restrain a Caterpillar pipe-laying tracker had only one wire rope clip installed. Among other methods, one feasible and acceptable method to correct this hazard would be to install the proper amount of wire rope clips in accordance with The Crosby Groups Workbook, Edition 6, Section IV-A, page 48, wire rope clips.
