Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards
that were causing or likely to cause death or serious physical harm to
employees in that the
company did not follow manufacturer's recommendations regarding the
maintenance of a
Lift-All aerial device:
On and about May 20, 2009, employees using a Lift-All Aerial Device, Model
Number
LAN-65-2E, Serial Number 5896039865LAN, were exposed to fall hazards
resulting from
chain failure and the company did not adhere to the critical inspections
and tests outlines in
the maintenance & repair parts manual from Lift-All Hoist in that the
chains were not
removed and cleaned for inspection to identify cracks.
American National Standard for Vehicle-Mounted Elevating and Rotating
Aerial Devices, ANSI/SIA A92.2 2001, paragraph 8.2.4, states:
"Periodic Inspection and Test:  An inspection of the mobile units shall be
performed at the intervals defined in 8.2.2 depending upon its activity,
severity
of service, and environment, or as specifically indicated below (These
inspections shall include the requirements of 8.2.3).
(1)  Structural members for deformation, cracks or corrosion."
Among other methods, one feasible and acceptable method to abate this
hazard is for the
employer to remove, clean, and inspect the chain per the manufacturer's
requirement.
ABATEMENT DOCUMENTATION IS REQUIRED FOR THIS ITEM IN ACCORDANCE
WITH THE REQUIREMENTS OF 29 CFR 1903.19(d).S
