Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to hazards associated with unstable loads due to a defective underslung hoist : On or about January 8, 2010, employees were exposed to the hazards of loads dropping due to a malfunctioning Coffing underslung crane, located outdoors near the south end of the grounds. The hook was missing the safety latch, the plastic covering the down button was partially missing, the pendant was taped up with non electrical duct tape, and the hoist failed to maintain the lifting heights during operations. One method of abatement, among others, would be to have this electric hoist replaced with one that meets the requirements established by ANSI B30.16-2007. There is no abatement certification or documentation required for this item.
