29 CFR 1904.29(b)(3): Each recordable injury or illness was not entered on the OSHA 300 Log and/or an incident report (OSHA Form 301 or equivalent) within seven (7) calendar days of receiving information that a recordable injury or illness has occurred: a) The employer did not document each work-related OSHA recordable injury/illness onto the OSHA 300 Log for year 2006. On or about 08/11/06, a Ron Williams Construction, Inc. employee was administered Intravenous (IV) Fluids in a hospital emergency room to treat heat cramps suffered at Calcasieu Refining Company. The incident, Case No. 06-046, was not entered on the OSHA 300 Log.
