Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to crushing/struck-by hazards:
(a) For employees working in the service department operating the ALM
2-post lift,
model #9001, serial #510-039, and exposed to the potential for injury
created by
excessive oil/grease allowed to accumulate on lifting chains/cables and/or
worn lifting
pads creating metal-to-metal contact between the lifting pads and
car/truck bodies
creating a potential unstable condition, an annual inspection and employee
training had
not been conducted
(b) For employees working in the service department operating the Nubaum
2-post lift,
serial #123182, and exposed to the potential for injury created by
excessive
oil/grease
allowed to accumulate on lifting chains/cables and/or worn lifting pads
creating metal-to-
metal contact between the lifting pads and car/truck bodies creating a
potential unstable
condition, an annual inspection and employee training had not been
conducted.
(c) For employees working in the service department operating the
Challenger 2-post lift,
serial #270569, and exposed to the potential for injury created by
excessive oil/grease
allowed to accumulate on lifting chains/cables and/or worn lifting pads
creating metal-to-
metal contact between the lifting pads and car/truck bodies creating a
potential unstable
condition, an annual inspection and employee training had not been
conducted.
(d) For employees working in the service department operating the VBM
2-post lift,
serial #272451, and exposed to the potential for injury created by
excessive oil/grease
allowed to accumulate on lifting chains/cables and/or worn lifting pads
creating metal-to-
metal contact between the lifting pads and car/truck bodies creating a
potential unstable
condition, an annual inspection and employee training had not been
conducted.
(e) For employees working in the quick lube department operating the
Benwil 2-post lift,
serial #2411, and exposed to the potential for injury created by excessive
oil/grease
allowed to accumulate on lifting chains/cables and/or worn lifting pads
creating metal-to-
metal contact between the lifting pads and car/truck bodies creating a
potential unstable
condition, an annual inspection and employee training had not been
conducted.
(f) For employees working in the quick lube department operating the Grand
4-post lift,
serial #1209056-I, and exposed to the potential for injury created by
excessive oil/grease
allowed to accumulate on lifting chains/cables, an annual inspection and
employee
training had not been conducted.
Abatement Note:
Among other methods, one feasible and acceptable method of abatement would
be to
follow the requirements of ANSI/ALI ALOIM and/or ANSI B153.1 which
includes,
among other requirements, the following:1.Institute a preventive
maintenance program according to the manufacturer's
specifications.
2.Provide training to employees on proper use and care of the lifts.
3.Ensure all lifts are rated and marked with their load capacity.
4.Schedule and complete all periodic and annual inspections.
