manufacturer was not obtained before
authorizing the modification to a CASE 585E 4 X 4 Fork Truck, no m/n or
s/n.  The modification was in the
form of a home-made personnel platform that was attached to the forks of
the truck and used by employee(s)
to install wooden soffit and fascia.
One method of abatement is to obtain written approval from the
manufacturer as required by ANSI B56.1-
1969, paragraph 4.2.1.
