Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to a struck-by and laceration hazard from falling glass: a)Employees were exposed to being struck by large sheets of glass and mirror in that the employees were required to stand between the mast of the Crown powered industrial truck and the glass and mirror. Employees walked, holding onto the large, overhead sheets of glass and mirror to help control the load and guide the glass and mirror onto glass processing machines, on or about 03/16/2009. Among other methods, one feasible and acceptable abatement method to correct this hazard is to utilize an attachment that positively secures the glass and mirror during transport without the employees holding the glass and mirror.
