from recognized hazards that were causing or likely to cause death or
serious physical harm
to employees in that employees were exposed to:
a.At the stringpiece, an employee was operating a powered industrial
truck without wearing a seat belt, which exposed the employee to
caught by/in between hazards, on or about 6/11/2009.
Among the feasible and acceptable methods to correct the hazard is to
prohibit employees from driving this type of equipment without wearing
the seat belt.

respectively.
