Employee(s) were exposed to a fall hazard of 80 ft. from a building on
which they
were performing pressure washing while suspended from boatswain chairs.
a. On or about 12/02/2009, employees' lifelines were not attached to
anchorages that had been
inspected annually by a qualified person as required by paragraphs 9.1.9.

b. On or about 12/02/2009, employees' lifelines were not attached to
anchorages that had been
certified under the supervision of a registered professional engineer as
required by paragraph
9.1.10 of the American National Standard/International Window Cleaning

Abatement methods.  The following feasible and acceptable methods to
correct these deficiencies
are:
a. To have a qualified person inspect the lifeline anchorages annually as
required by paragraphs
9.1.9. of the American National Standard/International Window Cleaning
Association Window
Cleaning Safety ANSI/IWCA I-14.1
b. To have the anchorages certified under the supervision of a registered
professional engineer
as required by paragraph 9.1.10 of the American National
Standard/International
Window
Cleaning Association Window Cleaning Safety ANSI/IWCA I-14.1.
PLEASE NOTE: DOCUMENTATION OF ABATEMENT IS REQUIRED FOR THIS
VIOLATION.
