Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized
hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to a fall hazard when being raised on a wooden pallet by a
material lift to access
the openings of a grain bin.
a) At the time of the inspection, a laborer and the foreman were raised in
an extension boom
basket engaged in sign construction duties.
The following problems were found:
1. No fall protection used when lifted to a height of 17 feet.
2. The outriggers on the boom extension lift truck were not fully extended
or evenly extended
as required.
4. The vehicle was improperly positioned as indicated by the vehicle
leveling device.  Outriggers
positioned on the curb tilted the lift vehicle away from the curb
unevenly.
Means of abatement include but are not limited to following the
manufacturer's instruction and
/or the standards as established by the Power Crane and Shovel Association
(PCSA) No. 2
section 7.01.1 directive.
Abatement certification and abatement documentation is required for this
violation.  The
documentation should include written verification of abatement, applicable
measurements or
monitoring results, and photographs or videos, which you believe will be
helpful.  The
abatement certification sheet is enclosed with the citation(s).
