Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The

furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to

were exposed to amputation hazards while operating hydraulic press brakes.
a) On or about July 30, 2009, Fabrication Department employees were
operating the Accurpress
250 ton hydraulic press brake, SN 7030, to fabricate Wall Panels without
protecting the
employees from entering the point of operation during the cycling of the
press brake.
b) On or about November 25, 2009, a Fabrication Department employee was
operating the
Accurpress 175 ton hydraulic press brake, Property Tag # 01132, to
fabricate Stiffeners without
protecting the employee from entering the point of operation during the
cycling of the press
brake.
Among other methods, one feasible and acceptable abatement method to
correct this hazard is
to follow the guidelines stated in American National Standard Institute
(ANSI) B11.3 - 2002,
Safety Requirements for Power Press Brakes, Section 8.4 Hazards associated
with point of
operation.
