Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees:
On or about May 3, 2011, two employees were working in the plant of
Steelmasters LLC in
Mayville, WI while a stress relieving oven used to harden steel was in
operation.  Both
employees were exposed to carbon monoxide in excess of the NIOSH Short
Term Exposure
Limit (STEL) of 200 parts per million (PPM) at 478 and 492 PPM,
respectively.  Inhalation of
carbon monoxide causes tissue hypoxia by preventing the blood from
carrying sufficient oxygen.
Symptoms of exposure may include headache, dizziness and nausea,
tachypnea, weakness,
confusion, hallucinations, cyanosis, angina, and syncope.
Among other methods, feasible and acceptable abatement methods include,
but are not limited
to adhering to NFPA 86 - 2011, increase make up air, modify or upgrade
existing duct work
to remove air, modify or upgrade the oven to not exhaust into the building
and operate and
maintain the furnace per the manufacturers requirements.
