(a)Date of Violation:08/20/09 Equipment:Yale Forklift, RFB542, Model: #3ERP040THN36TE082, Serial: #F807N02992C; Yale Forklift, RF2540B, Model: #ERCO40RGN48TE084, Serial: #EI08V18546A Condition:Employees were not wearing seat belts while operating electric forklifts. Among other methods, one feasible and acceptable abatement method to correct this hazard is to install and require the use of an operator restraint system in accordance with the American National Standards Institute (ANSI) B56.1-2005, Standard for Powered Industrial Trucks. Abatement documentation (photographs, receipts, copies of programs, training certification, etc.) is required for this item.
