fall/tip over hazards, at elevations of approximately 30 feet, while

a forklift basket that was not marked with workload and empty weight
markings
and had floor dimensions greater than the overall width of the truck
(measured
across load bearing tires plus 10 inches on either side).
ONE FEASIBLE AND ACCEPTABLE MEANS OF ABATEMENT WOULD BE TO:
Ensure the work platform is designed and constructed according to the
acceptable
requirements of the American National Standard Institute/Safety Standard
for
Rough
Terrain Forklift Trucks, ANSI B56.6-2002, Part III, Section 8.24.1,
platforms for
elevating personnel.
Abatement certification is required for this item.
