Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm in that employees were exposed to being struck by and/or crushed by rolls being handled by use of an overhead hoist: (a) MACHINE ROOM -- AN ANNUAL (PERIODIC) INSPECTION OF A 5-TON SHAWBOX, MODEL #72LOSO18D13, SERIAL #98J12752, HOIST #30, HAD NOT BEEN CONDUCTED SINCE 10/3/2005. Among other methods, a feasible and acceptable means to correct the hazard is to follow the manufacturer's/owner's manual recommendations for periodic inspection(s) of the above listed hoist, and/or to establish and follow a program of frequent and periodic inspection(s) of the hoist in accordance with ASME B30.16-2003, American Society of Mechanical Engineers for Overhead Hoists (Underhung), Section B30.16-2.1.5. ABATEMENT DOCUMENTATION is required as per 29 CFR 1903.19(c)(1) which requires certification that the abatement of the above violation is completed and 29 CFR 1903.19(d) requires documentation demonstrating that abatement has been completed.
