Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm in that employees were exposed to being struck by or crushed by load(s) being handled by hoist(s): (a) ESTABLISHMENT -- FREQUENT INSPECTIONS WERE NOT CONDUCTED ON THE FOLLOWING HOISTS: 1. BUILDING #2, SHIPPING/RECEIVING -- A 2-TON YALE, MODEL #KEL2-10R17-1/2 S2, SERIAL #K198034D HOIST; 2. MAIN BUILDING, 2ND TO 3RD FLOOR -- A 1-TON GIANT LIFT, MODEL 939, SERIAL #603-964-5127, HOIST; 3. MAIN BUILDING, CHIP SPINNER -- A 1/2-TON YALE, MODEL 3019YT,SERIAL # J 3619YT, HOIST; 4. MAIN BUILDING, SHIPPING/RECEIVING -- A 4-TON BUDGIT, MODEL 5215M, SERIAL #XL-5984-SQ, HOIST; 5. MAIN BUILDING, 1ST TO 2ND FLOOR -- A 1-TON GIANT LIFT, MODEL 939, SERIAL #6205-2R5, HOIST. Among other methods, a feasible and acceptable means to correct the hazard is to follow the manufacturer's/owner manual recommendations for frequent inspection(s) of each type of hoist used, and/or to establish and follow a program of frequent inspection(s) of the hoists in accordance with ASME B30.16-2003, American Society of Mechanical Engineers for Overhead Hoists (underhung), Section B30.16-2.1.5. ABATEMENT DOCUMENTATION IS REQUIRED. 29 CFR 1903.19(c)(1) requires certification that the abatement of the above violation is completed and 29 CFR 1903.19(d) requires documentation demonstrating that abatement has been completed.
