Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm in that
employee(s) were exposed
to a fall hazard from a work platform suspended on the forks of a forklift
truck:
(a) LUMBER YARD --  ON OR ABOUT 11/4/2009, AN EMPLOYEE WAS OBSERVED
WORKING FROM A WORK PLATFORM AT AN APPROXIMATE HEIGHT OF 10 FEET
FROM GROUND LEVEL, TOSSING STRAPS OVER A LOAD IN ORDER TO SECURE
THE WOODEN TRUSSES ON A FLATBED TRUCK WHICH WAS SCHEDULED TO BE
DELIVERED TO A CUSTOMER, EXPOSING SAID EMPLOYEE TO POTENTIAL
MULTIPLE FRACTURES, MULTIPLE INTERNAL INJURIES, AND/OR DEATH IN THE
EVENT OF A FALL.
Among other methods, one feasible and acceptable method to correct this
hazard is to use
a platform that meets the design requirements contained in American
National Standards
Institute Standard ANSI B56.1-2005, entitled "Safety Standard for Low Lift
and High Lift
Trucks". In particular, comply with the following provisions of the
standard:
1. Have a qualified engineer evaluate the structural integrity of the
platform to ensure that the
platform has a structural safety factor of not less than 3 to 1 (ANSI
B56.1-2005, paragraph
7.37.1(g)).
2. Provide a means to securely attach the platform to the lifting carriage
or forks of the fork
truck (ANSI B56.1-2005 paragraph 7.37.3(c)).
3. Ensure that the maximum including personnel and equipment, weight of
the empty platform,
and minimum capacity of the truck on which the platform can be used is
prominently displayed
on the platform.  Note: The combined weight of the platform load and
personnel cannot exceed
one-half the capacity of the fork truck on which the platform is used.

2005,paragraph 7.37.3(i)).

to being struck by a nail(s) from pneumatic nail gun(s):
(a) IN THE PRODUCTION AREA(S) --  EMPLOYEE(S) WHO ARE REQUIRED TO USE
PNEUMATIC NAIL GUN(S) IN ASSEMBLING WOOD TRUSSES, WERE EXPOSED TO
BEING INJURED BY ACCIDENTAL FIRING OF NAIL(S) DURING OPERATION.
AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE ABATEMENT
METHOD TO CORRECT THIS HAZARD IS TO COMPLY WITH THE PROVISIONS OF
AMERICAN NATIONAL STANDARD FOR POWER TOOLS, ANSI SNT-101-2002, SAFETY
REQUIREMENTS FOR PORTABLE, COMPRESSED-AIR-ACTUATED, FASTENER
DRIVING TOOLS, SECTION 8.4.2 FOR SAFE OPERATION OF SAID EQUIPMENT
AND/OR TO ADOPT THE BASIC SAFETY WORK HABITS AS PUT FORTH BY THE
INTERNATIONAL STAPLE, NAIL AND TOOL ASSOCIATION (ISANTA) WHICH WAS
DEVELOPED IN CONJUNCTION WITH THE CONSTRUCTION SAFETY COUNCIL AS
FOLLOWS:
1) CHOOSE A TRIGGER SYSTEM FOR YOUR SPECIFIC NEEDS;
2) KEEP YOUR FINGER OFF THE TRIGGER WHEN NOT WORKING;
3) ALWAYS WEAR APPROVED SAFETY GLASSES; AND
4) READ AND UNDERSTAND ALL SAFETY INSTRUCTIONS such as, but not limited to,
do not point the tool toward yourself or anyone else; do not operate at a
pressure above the
manufacturers' rating; do not depress the trigger unless the nosepiece of
the tool is directed onto
a safe work surface; do not carry the tool by its air supply hose and/or
while the trigger is depressed; do not overreach; keep proper footing and
balance.
SPECIFIC ABATEMENT DOCUMENTATION IS REQUIRED FOR THIS ITEM.
