Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of being struck by flying or whipping pipe sections. Jobsite: 99 Millers Falls Road Northfield MA 01360 a)Northfield Mountain "Upper Intake": Employee(s) working in the "Upper Intake" were exposed to being struck by temporary 8" HDPE piping. On or about May 17, 2010 an employee was fatally injured when struck by a 40' section of temporary 8" HDPE piping while dewatering the "Upper Reservoir". Among other methods, one feasible and acceptable abatement method to correct this hazard would be: Create procedures and safeguards to ensure the proper installation of the pipe, including effective anchoring systems based on the expected forces and ensure the system is used and that procedures are followed. SPECIFIC ABATEMENT DOCUMENTATION THAT THIS VIOLATION HAS BEEN CORRECTED MUST BE PROVIDED WITHIN 10 DAYS AFTER THE ABATEMENT DATE IN ACCORDANCE WITH 29 CFR 1903.19(d)(1).

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm in that employees were exposed to fall and caught-in/between hazards. Jobsite: 99 Millers Falls Road Northfield MA 01360 On 5/17/2010 at the Northfield Mountain "Upper Intake", An employee was permitted to ride in and work from the bucket of a Caterpillar 330 excavator while attempting to clean silt from the sump pump hole. Among others, one feasible and acceptable means of abatement would be to discontinue using the excavator bucket as a work platform or personnel carrier. SPECIFIC ABATEMENT DOCUMENTATION THAT THIS VIOLATION HAS BEEN CORRECTED MUST BE PROVIDED WITHIN 10 DAYS AFTER THE ABATEMENT DATE IN ACCORDANCE WITH 29 CFR 1903.19(d)(1).
