to crushing hazards associated with operating a hoist with a hook that had
a defective safety
latch:
At Machine #17
Employees were using an overhead hoist at machine 17 to load a mold into
the machine. Hoist
#3 (P & H, 2-ton, S/N 609485) had a hook with a safety latch that was
defective and did not
spring back to the safe position. The hoist was moving a mold that weighed
over one ton
exposing employees to crushing, amputation, and laceration hazards. One
feasible and acceptable
means, among others, of abatement would be to replace the hook or install
a new safety latch.
