29 CFR Section 5 (a) (1) of the Occupational Safety and Health Act of
1970: The employer
did not furnish employment and a place of employment which were free from
recognized
hazards that were causing or likely to cause death or serious physical
harm to employees in
that employees were exposed to falling hazards from an improperly set up
ladder:
In front of 1119 Riverdale St, on or about February 16, 2011, employees
were using an
aluminum 28 foot Louisville brand extension ladder to access the roof of
the building to
facilitate their snow removal activities. The ladder did not extend past
the roof surface they
were accessing exposing the employees to falls in excess of 16 feet when
they were
transitioning from the ladder to the roof when ascending and from the roof
to ladder when
descending the ladder (respectively).
Among other methods, one feasible and acceptable method to correct this
hazard includes,
but is not limited to the following:
Conform to ANSI (American National Standards Institute) A 14.5 2000 9.3.10
Access to or
From Upper Levels. When single section or extension ladders are used to
get on or off at
upper levels, the top of the ladder shall extend approximately three feet
above the point of
support at the eaves, gutter or roof line.
One feasible means of abatement would be to provide a course in ladder
safety to all
employees who use ladders as a necessary part of their job duties.
