employer did not furnish each of his employees conditions of employment
and a place of employment free from
recognized hazards that were causing or likely to cause death or serious
physical harm to employees in that
employees were exposed to struck-by hazards:
a)job site- where an employee was observed riding on the outside of the
cab of a Ford F-650
dump truck (licence plate: AC-8098) while standing on the step below the
drivers door and
holding on to the rear-view mirror and the climb assist handle.
ABATEMENT NOTE: One feasible abatement, among others, would be to re-train
employees
in proper vehicle operation and to require employees to wear seatbelts
inside the truck cab
between stops.
