that employees were exposed to the hazard of being crushed by the weight
of a vehicle due to lift failure:
a)facility, bay 1 - the Rotary, model PSL6, Straddle, Low Rise Car Lift,
6,000 pound capacity,
was not provided with functional locking device.
ABATEMENT NOTE:
1)Among other methods, one feasible and acceptable abatement method to
correct these hazards
is to repair or replace the broken locking device.
2)Remove the lift from service and use only the lifts in bays 2 and 3.
