Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer did not
furnish employment and a place of employment which were free from
recognized hazards
that were causing or likely to cause death or serious physical harm to
employees in that
employees were exposed to fall hazards from improper ladder use:
a)Fabrication Room: On or about February 2, 2010, two workers were working
on a
mezzanine being accessed by stepping on the top plate of an 8 foot A-frame
fiberglass
ladder, exposing them to an approximate 8'2" fall to concrete below.
One feasible and acceptable abatement method, among others, to correct
this hazard is to
provide and require the use of a taller A-frame or extension ladder. If
another A-Frame
ladder is used, comply with the manufacturer's instructions and ANSI 14.5
for Ladders.  In
section 9.3.2 Climbing and Working Locations, it states: The user shall
not step or stand on:
(1) a ladder top cap and the top step of a step or trestle ladder, or the
bucket
or pail shelf of
a self supporting ladder.
