Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were likely to cause death or serious physical harm to the employees in that employees were exposed to struck by hazards: (a)S.W. Jack Drilling, Rig #36, Dingess Run #14 well site - Employees were exposed to struck by hazards while walking and working around flexible high pressure air lines that did not have whip check devices installed, as determined on 08/25/09. NOTE: Among other methods, one feasible and acceptable abatement method to correct this hazard is to follow accepted safe practices such as but not limited to; a) secure the hose using devices properly located and of sufficient strength to resist hose whipping should the hose become disconnected or broken, b) follow industry practice that secures flexible hoses with pressure greater then 100 psig, or c) follow the recommendation in API-RP54, Recommended Practice for Occupational Safety for Oil and Gas Well Drilling and Servicing Operations, Section 9.13. Pressure Equipment, to properly secure flexible hoses to prevent movement. ABATEMENT CERTIFICATION REQUIRED FOR THIS ITEM
