hazard of crushing injuries associated with the rollover of a lift truck
without the use of an
operator restraint system:
(a)Facility Lot: On November 17th 2009 and times prior to, an employee
operated
a Hyster fork lift (model # H30XM) without the use of the installed
seatbelt.
One feasible and acceptable abatement method to correct this hazard is to
follow
ASME B56.1-1993 Safety Standard for Low and High Lift Trucks, which
requires the usage of an operator restraint system at all times.
