Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer did not
furnish employment and a place of employment which were free from
recognized hazards
that were causing or likely to cause death or serious physical harm to
employees in that
employees were exposed to being sprayed by steam while working on a steam
station:
a.R.H. Lapp Mechanical Contractors: Energy control procedures were not
developed and implemented for use prior to employees working on a steam
station, consisting of a pump and receiver, without locking out the ball
valve
that shut off the steam, preventing the employee from inadvertently coming
in
contract with the hazardous energy/steam, as determined on or about
06/02/10.
NOTE: Among other methods, one feasible and acceptable abatement method to
correct this
hazard would be to comply with the American National Standard A10.44-2006
Control of
energy Sources (Lockout/Tagout) for construction and demolition Operations
and establish a
program consisting of energy control procedures, employee training, and
periodic inspections
to ensure that before any employee performs any servicing or maintenance
on a machine or
equipment, where unexpected energizing, start up or releases of stored
energy could occur
and cause injury, the machine or equipment shall be isolated from the
energy source and
rendered inoperative.
