that were causing or likely to cause death or serious physical harm to
employees in that
employees were exposed to the hazard of crushing injuries associated with
the rollover of a
lift trucks without the use of an operator restraint system:
(a)Site Wide: The operator of the Gehl rough terrain forklift (Co. Number
663)
was operating the lift with out utilizing the seat beat as observed on
7-30-10.
ABATEMENT NOTE: Among other methods, feasible and acceptable
abatement methods to correct this hazard are:
1. To follow ANSI/ASME B56.6-2005 Safety Standard for Rough Terrain
Forklift Trucks and require the usage of an operator restraint system at
all
times.
2. To follow the manufacturers operating warnings and wear the provided
seat
belts at all times when operating the lift trucks.
