Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer did not
furnish employment and a place of employment which were free from
recognized hazards
that were causing or likely to cause death or serious physical harm to
employees in that
employees were exposed to the hazard of crushing injuries associated with
the rollover of a
lift trucks without the use of an operator restraint system:
(a)Delivery Area:  On July 30, 2010 and times prior to, an employee
operated a
Princeton Piggy Back -PB11 SN#4241205 without the use of the installed seat
belts.
ABATEMENT NOTE: Among other methods, feasible and acceptable
abatement methods to correct this hazard are:
1.To follow ASME B56.1-1993 Safety Standard for Low and High Lift
Trucks and require the usage of an operator restraint system at all times
and to follow the manufacturer's operating manual. Also ensure the
operators wear the provided seat belts at all times while operating the
lift truck.
