Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of crushing injuries associated with the rollover of a lift trucks without the use of an operator restraint system: (a)Site Wide: On September 9, 2010 and times prior to, an employee operated a Ingersoll/Rand UR-60B-570B rough terrain extend boom forklift SN#144061 without the use of the installed seatbelts. ABATEMENT NOTE: Among other methods, feasible and acceptable abatement methods to correct this hazard are: 1. To follow ASME B56.6-2005 Safety Standard for Rough Terrain Forklifts Trucks and require the usage of an operator restraint system at all times. 2. To follow the manufacturers operating manual and wear the provided seat belts at all times when operating the lift trucks. ABATEMENT CERTIFICATION REQUIRED FOR THIS ITEM
