Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fall hazards: (a)On or about 20 July 2009 - Fabrication Shop, the assembly area of the fabrication shop the extension ladder did not extend three feet above the landing that was being accessed. (a) Among other methods, one feasible and acceptable method to correct this hazard is to extend the fiberglass extension ladder three rungs above the landing. (b)On or about 20 July 2009 - Fabrication Shop, an employee was not using the stepladder in the open position as required by the manufacturer. (b) Among other methods, one feasible and acceptable method to correct this is to use the stepladder in the open position as required.
