Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to struck-by hazards:
a)On November 20, 2009 at the jobsite located on the west side of Main
Street near
1634 East Main Street in Warsaw, MO, two employees were struck by flying
debris when a 6" high density plastic endcap blew off the end of a 6" gas
main
line.  The endcap then struck a pile of rocks and debris that subsequently
flew
into the employees, injuring two.  The endcap and gas main line were
connected
to a compressed air source without the use of a lockout/tagout system which
allowed the gas main line to be pressurized before the coupler holding the
endcap
to the main line had enough time to cool sufficiently following the heat
fusing
process.
AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE METHOD TO
CORRECT THIS HAZARD IS:
Institute a lockout/tagout program that addresses the hazards of
compressed air and all other
hazardous energy sources related to performing the air leak pressure test
on gas main lines.
This program should be in compliance with ANSI 244.1 2003 Control of
Hazardous Energy
Lockout/Tagout and Alternative Methods.
Abatement certification is required for this item-use the CERTIFICATION OF
CORRECTIVE
ACTION WORKSHEET for this purpose.T
