Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to a crushing hazard:
(a)At 10985 South Highway 169, Smithville - A JLG rough terrain forklift
was used
to lift an electric slab scissor lift. An employee was in the scissor lift
popping
rivet joints of the gutter while it was being lifted by the JLG. The
employee also
rode in the scissor lift as it was being transported by the JLG rough
terrain
forklift.
Feasible and acceptable methods to correct this hazard, among others,
include the
following:
(1)Per the JLG Operation & Safety Manual, when lifting personnel,
only use a JLG approved work platform.
(2)Per the JLG Operation & Safety Manual, do not drive the machine
from the cab when personnel are in the platform.
(3)Per the MEC Operation & Safety Manual, operate the scissor lift
only on even and solid ground.
(4)Per the MEC Operation & Safety Manual, use the machine
(electric slab scissor lift) only for purposes for which it was
designed.
