Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were not properly trained in the use of the Sala Exofit harness and
lanyard personal fall arrest
systems which they were provided with for use in an articulating boom lift:
a)On October 28, 2009 at the jobsite located at 1911 South Springfield
Avenue in
Bolivar, MO, an employee was observed working at height in the basket of
an articulating boom
lift.  The employee was wearing a full body harness with a 6' lanyard
attached
to the basket of
the lift.  The employee connected said lanyard to a side "D" ring rather
than to the "D" ring
located on the back between the shoulders, as indicated by the
manufacturer's instructions.
AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE METHOD TO
CORRECT THIS HAZARD IS:
To train the employee on the proper use of the fall arrest equipment in
accordance with
manufacturer's instructions.
Abatement certification is required for this item-use the CERTIFICATION OF
CORRECTIVE
ACTION WORKSHEET for this purpose.T
