employer did not
furnish employment and a place of employment which were free from
recognized hazards
that were causing or likely to cause death or serious physical harm to
employees in that
employees were exposed to crushing hazards as a result of being struck-by
a load that fails.
(a)Kodiak Builders, Inc. & 18335 East 103rd Ave., Commerce City, CO
80022: On and before February 6th, 2010, the exposing employer did not
ensure the forklift operator lowered the boom and the material on the
forks of
the rough terrain forklift prior to leaving the forklift, in that it is
allowed and
common practice to leave the forklift in this condition and perform other
work
greater than 25 feet from the forklift. This condition exposed the
employees to
the hazards of unsafe forklift operation.
Abatement Note: Among other methods, one feasible and acceptable abatement
method to
correct this hazard is to follow ANSI/ASME B56.6-1992, "Safety Standard
for Rough
Terrain Forklift Trucks and High Lift Trucks", paragraph 6.2.11 states, "A
rough terrain
forklift truck is unattended when the operator is 25 ft (7.6m) or more
from the truck, which
remains in his view, or whenever the operator leaves the truck and it is
not in his view."
Paragraph 6.2.12 Before leaving the operator's position:
(a)bring rough terrain forklift truck to a complete stop;
(b)place directional controls in neutral;
(c)apply parking brake;
(d)lower load-engaging means fully, unless supporting an occupied elevated
platform.
In addition, when leaving the rough terrain forklift truck unattended:
(e)stop the engine;
(f)if the rough terrain forklift truck must be left on an incline, block
the wheels;
(g)fully lower the load-engaging means.
