Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards
that were causing or likely to cause death or serious physical harm to
employees in that the
employer did not include all required elements on the confined space entry
permit:
(a)JBS Five Rivers Ranch Cattle Feeding LLC, at 38002 County Road N,
Yuma, CO 80759:  The employer did not ensure that the entry permit that
documented compliance and authorized entry to a permit required confined
space included information of the emergency services that would be summoned
and the means for summoning those services.  This condition exposed the
employees to hazards associated with potentially hazardous atmosphere in a
confined space.
(b)JBS Five Rivers Ranch Cattle Feeding LLC, at 38002 County Road N,
Yuma, CO 80759:  The employer did not ensure that the entry permit that
documented compliance and authorized entry to a permit required confined
space included information to identify the communication procedures used by
authorized entrants and attendants to maintain contact during the entry.
This
condition exposed the employees to hazards associated with potentially
hazardous atmosphere in a confined space.
