Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to an ammonia inhalation hazard: (a)Reddy Ice Group Inc, at 2101 31st Street, Denver, CO: On and preceding 9/14/10, Reddy Ice Group Inc did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to an ammonia inhalation hazard. The employer maintained approximately thirteen thousand pounds of ammonia in a closed loop refrigeration system. The control switches for the two Dayton wall fans and two Dayton ceiling fans mechanical exhaust system were located within the machine room. The employer did not ensure that the control switches for the four mechanical exhaust fans were located immediately outside the machine room door. (b)Reddy Ice Group Inc, at 2101 31st Street, Denver, CO: On and preceding 9/14/10, Reddy Ice Group Inc did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to an ammonia inhalation hazard. The employer maintained approximately thirteen thousand pounds of ammonia in a closed loop refrigeration system. The two Dayton wall fans located within the machine room were on the same electrical circuit as the machine room lighting. The employer did not ensure that the exhaust fans were on a separate electrical circuit from other equipment. Abatement Note: Abatement of this item may be achieved through compliance with American Society of Heating, Refrigerating and Air-Conditioning Engineers (ASHRAE) Standard 15-2007 "Safety Standard for Refrigeration Systems" section 8.12(i). The employer maintained approximately thirteen thousand pounds of ammonia in a closed loop refrigeration system. A mechanical integrity inspection performed in 2007 identified the pressure relief valves, V1-06 and V1-07, of V-1 high pressure receiver to be in excess of five years old. The employer's Process Hazard Analysis stipulated a five year replacement schedule for pressure relief valves. The pressure relief valves of V-1 high pressure receiver were the original valves installed in 1954. The employer did not document that the pressure relief valves complied with recognized and accepted good engineering practices. This condition exposed employees to an ammonia inhalation hazard. Abatement Note: Abatement of this item may be accomplished by compliance with International Institute of Ammonia Refrigeration Bulletin 110 "Guidelines for Start-up, Inspection and Maintenance of Ammonia Refrigerating Systems" section 6.6.3 which requires pressure relief valves to be removed and replaced with new or with overhauled and recalibrated valves at least every five years. The employer maintained approximately thirteen thousand pounds of ammonia in a closed loop refrigeration system. The isolation valve V1-08 associated with the pressure relief valves, V1-06 and V1-07, of V-1 high pressure receiver has a frozen stem and does not operate as designed. The operation of the isolation valve is necessary during the replacement of pressure relief valves V1-06 and V1-07. The employer did not correct the deficiency that had been identified during maintenance operations. This condition exposed employees to an ammonia inhalation hazard. Employees used Certified Labs Tekusolv II (containing d-limonene, 1-tetradecene, 1-dodecene, 1-octadecene, and 1-hexadecene) when cleaning parts in the parts washer. The employer provided nitrile disposable exam type gloves to wear when cleaning parts. Nitrile disposable exam type gloves do not provide adequate protection from exposure to organic solvents. This condition exposed employees to a chemical hazard. Abatement Note: Abatement certification is required for this item (See enclosed "Sample Abatement-Certification Letter").-Certification
