Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer did not
furnish employment and a place of employment which were free from
recognized hazards
that were causing or likely to cause death or serious physical harm to
employees in that
employees were exposed to respiratory hazards:
(a) Central Denver Ironworks, Inc., 4245 Fox St., Denver, CO 80216: On and
before 7/14/10, the employer did not ensure the CMS-2 Carbon Monoxide
Monitor was located in a way that a visual or audible alarm would
effectively
notify the employee of the alarm status. The CMS-2 was located on the media
blasting equipment and faced away from the operator who, when equipped
with a supplied air respirator hood and while operating media blasting
equipment would not be alerted to the alarm condition. This condition
exposed
employees to respiratory hazards.
Abatement note: Among other methods some feasible and acceptable abatement
methods to
correct this hazard is to perform the following:
1.Mount the CMS-2 on a mobile base in according to manufacturer's
recommendations;
2.Use the auxiliary terminals and rely to interlock the CMS-2 to other
devices,
such as the compressor shut-down mechanism; and/or
3.Station an employee, who is trained on the CMS-2, to alert the employee
equipped with the supplied air respiratory system of the alarm status.
Abatement note: Abatement certification and documentation is required for
this item (See
enclosed "Abatement Certification Letter").
