At the Norm Baker Motor Company service facility, the employer did ensure
that the following
processes were implemented and maintained: in accordance with the American
National
Standards Institute/ American Lift Institute Safety Resquirements for
Operation,
Inspection and
Maintenance (ANSI/ALI ALOIM - 2008):
a.Operators of automotive lifts were not instructed in the safe use and
operation of
the lifts per the ANSI/ALI ALOIM - 2008, page 6, para. 4.2.1. There were no
operating instructions available for employee reference regarding  eleven
10,000
Ib. load capacity SPX Stone Floor Lifts.
b.Operating instructions were not displayed in a conspicuous location in
the lift area
per ANSI/ALI ALOIM - 2008, page 6, para. 4.2.2.
c.Operator training was not documented by completing the operator training
log
found in ANSI/ALI ALOIM - 2008, page 6, para. 4.3.
d.Daily Inspections. The employer did not inspect all the required items
during the
inspection process in accordance with ANSI/ALI ALOIM - 2008, page 6, para.
4.4.4(1). No daily inspection documentation was provided. The eleven
10,000 Ib.
load capacity SPX Stone Floor Lifts did not have operator manuals
containing
inspection information.
e.A qualified lift inspector did not follow the specific directions
provided by the lift
manufacturer in the inspection of each lift per ANSI/ALI ALOIM - 2008,
page
8, para. 5.6.1. There were no manufacturer supplied checklists or
inspection
certificates for each lift (14) signed by the qualified lift inspector and
employer
representative and posted along with the inspection checklist on or near
the lift
inspected or together with other printed material or records pertaining to
the lift.
f.Planned Maintenance Procedure. The employer did not establish a periodic
planned maintenance procedure in accordance with the recommendations of the
lift manufacturer in order to ensure reliability and allow the continued
safe
operation of the lift (ANSI/ALI ALOIM - 2008, page 12, para. 6.1.1).
