SCRR 71-112A: Failed to furnish a place of employment which is free of recognized hazards which may cause death or serious physical harm to his employees and comply with this regulation and other occupational safety and health rules and regulations promulgated under Chapter 15 of Title 41, Code of Laws, State of South Carolina, 1976, as amended,0 as follows: A. Employer knew or should have known that employee operating a Genie forklift, model GTH-644, equipped with a roll over protection system without wearing the seatbelt was exposed to the hazard of being thrown from the forklift at the Players Village condos job site. A feasible and useful method, among others, to correct this hazard is to ensure employees wear the seatbelt when operating the forklift.

SCRR 71-112A: Failed to furnish a place of employment which is free of recognized hazards which may cause death or serious physical harm to his employees and comply with this regulation and other occupational safety and health rules and regulations promulgated under Chapter 15 of Title 41, Code of Laws, State of South Carolina, 1976, as amended,0 as follows: A. Employer knew or should have known that two employees elevated in excess of 20 feet in a personnel platform on a Genie forklift, model GTH-644, with no operator at the controls constituted a hazard of no means of egress if an emergency occurs. A feasible and useful method to correct this hazard, among other methods is to ensure the operator stays at the controls of the forklift while employees are in personnel platform.
