Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer did not furnish
employment and a place of employment which were free from recognized
hazards that were
causing or likely to cause death or serious physical harm to employees in
that:
(a)Workers in the scrap wood processing shop were potentially exposed to
the
hazard of a wood dust deflagration/explosion which could propagate from the
Steelcraft dust collector through the inlet ducts (which normally conveyed
dust-
laden air from the shop out to the dust collector) and back into the shop.
ABATEMENT NOTE: Among other methods, one feasible and acceptable abatement
method
to correct these hazards is to comply with the National Fire Protection
Association (NFPA) 664,
"Standard for the Prevention of Fires and Explosions in Wood Processing
and Woodworking
Facilities" (2007 Edition) as it applies to your operations and equipment,
including but not
limited to the following:
* Isolate the conveying system to prevent propagation of deflagration
upstream into occupied
areas in accordance with Section 8.2.4.  Among other feasible means of
abatement, back blast
damper(s) could be installed on the inlet ducts (which normally convey
dust-laden air from the
shop out to the dust collector).
Certification, with date and method of abatement required.
Certification of abatement is required by 29 CFR Part 1903 and is to be
submitted within ten (10)
calendar days after the abatement date.ment

exposed to potential wood dust deflagration and explosion hazards caused
by improper design
and maintenance of the scrap wood processing shop:
(a)The Bliss Hammermill, in which scrap wood was processed, was located
inside
the shop where workers performed various tasks.  Because the wood dust
inside
the hammermill was explosive and the potential existed for ignition of
that dust,
workers in the shop were potentially exposed to the hazards of
deflagration and
explosion.
Abatement Note: Among other methods, one feasible and acceptable method to
correct these
hazards is to comply with National Fire Protection Association (NFPA) 664,
"Standard for the
Prevention of Fires and Explosions in Wood Processing and Woodworking
Facilities" (2007
Edition) as it applies to your operations and equipment including but not
limited to the
following:
* Locate the hammermill outdoors in accordance with Section 8.4.2.4.1.
(Alternatively, locate the hammermill in a detached building or separated
from
other production
areas by damage-limiting construction.)
Certification, with date and method of abatement required.
Certification of abatement is required by 29 CFR Part 1903 and is to be
submitted within ten (10)
calendar days after the abatement date.ment

exposed to potential wood dust explosion, deflagration, and fire hazards
caused by inadequate
plant "blow down" procedures:
(a)In the shop area, the following deficiencies were found with "blow down"
procedures:
(1) "Blow down" was performed with electric powered metal halide lights and
associated wiring and receptacles energized.
(2) Air hoses for "blow down" were greater than 15 psi gauge pressure.
ABATEMENT NOTE: Among other methods, one feasible and acceptable method to
correct this hazard would be to ensure workers minimize the generation of
dust in
accordance with National Fire Protection Association (NFPA) 664 "Standard
for the
Prevention of Fires and Explosions in Wood Processing and Woodworking
Facilities"
(2007 edition) as it applies to your operations and equipment, including
but not limited
to the following:
* Perform removal of dust in accordance with Sections 11.2.1.1 and
11.2.1.2.
Certification, with date and method of abatement required.
Certification of abatement is required by 29 CFR Part 1903 and is to be
submitted within ten (10)
calendar days after the abatement date.ment
