had the pendant had electrical tape wrapped around the electrical wires.
The
pendant was not supported in a manner that would protect the electrical
conductors exposing employees to electrical hazards.
(b)In the Machine Shop by the Drill Press & Chucker Area, Budgit  Ton Jib
Hoist
pendant was not supported in a manner that would protect the electrical
conductors.
(c)In the Machine Shop by Warner & Swasey Automatic Chucker,  Ton Capacity
Lodestar Hoist pendant was not supported in a manner that would protect the
electrical conductors.
Among other methods, feasible and acceptable means to correct these
hazards are to use a safer
alternate method such as ensure the pendant control was properly supported
as outlined in ASME
Overhead Hoist (Underhung) B30.16-2003, Section 16-1.2.3(d) a pendant
station shall be
supported in a manner that will protect the electrical conductors or air
hoses against strain.  Any
pendant station that might present a hazard to the operator, if a ground
fault occurs, shall be
grounded.
Certification, and documentation/evidence of abatement is required.

rated load and other identification.  This exposed employees to struck by
hazards
from falling loads.
(c)In the High Bay, the employer had several job made lifting devices
(i.e. spreader
bar and hooks) used to move various parts without having the rated load and
other identification.  This exposed employees to struck by hazards from
falling
loads.
Among other methods, feasible and acceptable means to correct these
hazards are to use a safer
alternate method such as:
Follow the American National Standard ASME B30.20-2001 Below the Hook
Lifting
Devices, Section 20-1.2.1(a) Rated Load.  The rated load of the lifting
device shall be
marked on the main structure where it is visible.  If the lifting device
is made up of
several lifters, each detachable from the group, these lifters shall also
be marked with
their individual rated loads.
Follow the American National Standard ASME B30.20-2001 Below the Hook
Lifting
Devices, Section 20-1.2.1(b) Identification.  A nameplate or other
permanent marking
shall be affixed displaying the following information:  manufacturer's
name and address;
serial number; lifter weight, if over 100 lb (45 kg); rated load.

2") was used a move a steel plate 1u" thick.  This clamp was not used in
accordance with the instruction manual in that it was used for vertical
lifts and
turned into a horizontal lift.
Among other methods, feasible and acceptable means to correct these
hazards are to use a safer
alternate method such as follow the requirements as outlined in:
American Society of Mechanical Engineers ASME B30.20a-2001 Below-th-Hook
Lifting Device:
20-1.4.4(b)  The lifting device shall not be loaded in excess of its rated
load or handle
any load for which it is not designed.
20-1.4.4(c)  The lifter shall be applied to the load in accordance with
the
instruction
manual.
Renfroe Model S Clamp Operation and Maintenance Manual:
Application S, The model "S" is a vertical lifting clamp and is an
effective tool for
construction and erection.
Operation S, Step 2, Select appropriate capacity and plate thickness. The
model
designation, capacity, and plate thickness are stenciled on each clamp.
WARNING:
Never exceed rated capacity or use on plates that are not within the range
of plate
thickness stenciled on the clamp.  Lift only one plat on each lift.
Always use a clamp
with maximum plate thickness and rated capacity near equal to the
thickness and weight
of the plate being lifted.
Operation S, Step 8, Commence lift.  WARNING:  The operator should position
himself away from and fully clear of the member to be lifted.  Do not
commence
lift
until all personnel are clear of the area of the lift.  Never stand under
or near a
member being lifted.
Operating Aid, 6.  DO Use clamps within their rated capacity.  NEVER
overload clamp.
Certification, and documentation/evidence of abatement is required.
