Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer did not furnish
employment and a place of employment which were free from recognized
hazards that were
causing or likely to cause death or serious physical harm to employees in
that inspection and
maintenance of the Ammonia Mechanical Refrigeration Systems is not
implemented as required,
exposing employees to chemical burns and inhalation hazards from release
of ammonia, and
struck by hazards from over pressure failure of ammonia refrigeration
components:
(a)Ammonia Mechanical Refrigeration Systems: The employer has not
developed an
Inspection and Maintenance Program to assess the mechanical integrity of
the four
seperate ammonia refrigeration systems. Mechanical systems include but is
not
limited to compressors, pressure vessels, heat exchangers, pumps, valves,
piping
and protection devices and sensors.
(b)West/Central Freezer Compressor Room Accumulator: Access to liquid
ammonia
pumps 1 and 2 and adjacent valves were not readily accessible in-that the
pumps
and isolation/shutoff valve hand wheels were encapsulated with excessive
ice/frost
build-up and did not allow immediate access to the pump isolation/shutoff
valves.
A feasible means of abatement includes, but is not limited to meeting
requirements of the International Institute of Ammonia Refrigeration (IIAR)
Bulletin No. 110 Start-Up, Inspection, and Maintenance of Ammonia
Mechanical
Refrigeration Systems, Section 6 Inspection and Maintenance of Ammonia
Refrigeration Systems.
Certification, and documentation/evidence of abatement is required.
Certification of abatement is required by 29 CFR Part 1903 and is to be
submitted within ten (10)
calendar days after the abatement date.ment
