Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that: (a)At the Lot 2 S. 94th St., West Allis jobsite; the elevated platform was not secured to the lifting mechanism exposing employees to tipping hazards. Among other methods, feasible and acceptable means to correct these hazards are to use a safer alternate method such as: Follow ASME B56.6-2002 Section 5.15.1(b) be certain that the platform is securely attached to the lifting carriage and forks. Certification, and documentation/evidence of abatement is required. Certification of abatement is required by 29 CFR Part 1903 and is to be submitted within ten (10) calendar days after the abatement date.ment

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to death and serious injuries from a substandard suspended unguarded platform: (a)At the Lot 2 S. 94th St., West Allis jobsite; an elevated platform, used to install soffit, exceeded two times the load center distance exposing employees to tipping hazards. Among other methods, feasible and acceptable means to correct these hazards are to use a safer alternate method such as: ensure the platform conforms to the requirements as outlined in ASME (American Society of Mechanical Engineers) B56.6-2002 Section 8.24.1 (b) floor dimensions which shall not exceed two times the center distance listed on the rough terrain forklift truck nameplate, measured parallel to the longitudinal center plane of the truck, nor have a width greater than the overall width of the truck [measured across the load bearing tires plus 10 in. (250 mm) on either side]. Certification, and documentation/evidence of abatement is required. Certification of abatement is required by 29 CFR Part 1903 and is to be submitted within ten (10) calendar days after the abatement date.ment
