Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to an exploding boiler hazard while working on or near the
boiler located in the
production area.
a) At the time of the inspection and up to a year prior the boiler
secondary shut off was by-
passed in order to get the boiler to operate without shutting off. The
following problems were
found;
b) There was no secondary shut-off that would prevent over-pressure
build-up in the boiler as
required in the ASME Boiler and Pressure Vessel Code, Section IV-1974,
Heating Boilers,
Article 6, Instruments, Fittings and Controls, HG-604 Pressure Control
which states: "Each
automatically fired steam boiler shall be protected from over pressure by
two
pressure operated
controls."
Among other methods, feasible and acceptable abatement means to correct
these haards include,
but are not limited to:
Replace the faulty secondary shutoff switch with a fully functional switch
and eliminate the by-
pass wiring.
Abatement certification and abatement documentation is required for this
violation.  The
documentation should include written verification of abatement, applicable
measurements or
monitoring results, and photographs or videos which you believe will be
helpful.  The abatement
certification sheet is enclosed with the citation(s).
