Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to the condition(s) listed below:
JBS USA, LLC, 5950 Trails End Road, Cactus, Texas. For the period of time
up to and
including April 13, 2010, an employee was performing work operations such
as replacing light
bulbs while elevated by a basket on Caterpillar forklift. Employee exited
the basket to tie off on
a hoist beam to reach the light and fell approximately 20 feet or more to
the floor. This
condition exposed employee to fall hazards.

for this item.
