(a)Shop Area - On or about 9/15/09 employees were allowed to operate a Yale
forklift which did not have seat belts.
Among other methods, a feasible and acceptable method of correction is to
install a seat belt
retro-fit package and require seat belts to be worn when the forklift is
in operation.
