Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to a trip hazard that was caused a floor pipe removal. (a). In front of press #18 - On or about January 21, 2010 a hole was in the floor in front of press # 18. A pipe was removed from the floor and the hole caused by the pipe removal was not filled in. The hole was 6 inches wide, 6 1/2 inches long and 3 3/4 inches deep. An employee could stepped in the hole and twist an ankle, knee or trip and fall and fracture an arm.
