Section 5(a)(1) of Public Law 91-596, the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that: equipment was removed, but the anchor bolts and footings were left attached to the floor, which created a tripping hazard. (a)Upstairs deink #1 - On or about June 8, 2010 a shaker was removed from upstairs deink #1 and the anchor bolts and footings were not removed. The anchor bolts and footings protruded upward 2 inches, which created a trip hazard. Among feasible and acceptable abatement methods to correct this hazard is to remove the anchor bolts and footings or cut the anchor bolts and footings down to floor level.
